Home Health LPN | One-on-One Patient Care
About The Position
SummitWest Care is a local nonprofit home health agency seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled nursing services throughout Mesa County. Home health offers a different nursing experience. Instead of balancing multiple call lights, constant interruptions, and large patient assignments, you'll provide one-on-one care where it matters most: in the patient's home. You'll have the flexibility to build your schedule, the autonomy to work independently, and the support of an experienced clinical team committed to your success. Whether you're seeking part-time, PRN, or supplemental hours, this role offers the opportunity to build meaningful patient relationships while helping individuals remain safe, healthy, and independent at home.
Requirements
- Current Colorado Licensed Practical Nurse (LPN) License
- Valid driver's license, reliable vehicle, and auto insurance
- Strong communication and accurate documentation
- Ability to work independently and adapt professionally in home health environments
Nice To Haves
- IV Certification preferred; not required
Responsibilities
- Provide skilled nursing services in patient homes
- Perform wound care, medication support, and patient monitoring
- Observe and report changes or concerns promptly
- Document visits accurately and timely in the EMR
- Collaborate with RNs, therapists, caregivers, families, and providers
- Support patient education and individualized care plan goals
