Physical Therapist Assistant (PTA) | One-on-One Home Health Care
About The Position
SummitWest Care, a local nonprofit home health agency, is seeking a compassionate Physical Therapist Assistant (PTA) to provide one-on-one therapy services throughout Mesa County under the direction of a licensed Physical Therapist. Home health offers a different therapy experience, allowing for focused patient progress, meaningful relationship building, and direct observation of results. The role offers flexibility and independence in the field, supported by experienced therapists, clinical leadership, and office staff. The position is available for both full-time and part-time hours, and the team is dedicated to helping patients improve mobility, safety, independence, and quality of life at home.
Requirements
- Current Colorado Physical Therapist Assistant (PTA) license in good standing
- Graduate of an accredited Physical Therapist Assistant program
- Valid driver's license, reliable transportation, and auto insurance
- Strong communication and documentation skills
- Ability to work independently and adapt professionally in home-based setting
Responsibilities
- Provide skilled therapy services in patient homes
- Carry out established treatment plans involving therapeutic exercise, gait training, balance activities, and functional mobility intervention
- Monitor and communicate patient progress and changes to supervising PT
- Encourage patient participation and reinforce goals
- Complete timely, accurate documentation in EMR
- Collaborate with therapists, nurses, caregivers, and other care team members
