SummitWest Care is seeking a compassionate Physical Therapist to provide one-on-one care throughout Mesa County. Requirements

  • Current Colorado Physical Therapist (PT) license in good standing
  • Graduate of an accredited Physical Therapy program
  • Valid driver's license, reliable transportation, and auto insurance
  • Strong clinical assessment and treatment skills
  • Excellent communication and documentation abilities
  • Ability to work independently while collaborating effectively with an interdisciplinary team
  • Ability to adapt professionally in a home-based care environment

Responsibilities

  • Evaluate patients and develop individualized, person-centered care plans
  • Provide skilled physical therapy services in patient homes
  • Improve patient mobility, strength, balance, and functional independence
  • Educate patients and caregivers on treatment goals and home exercise programs
  • Monitor patient progress and modify treatment plans to support individual goals
  • Complete accurate and timely clinical documentation
  • Collaborate with nurses, therapists, physicians, caregivers, and other members of the care team to achieve the best possible outcomes
