About The Position
Alpha Home Health & Hospice is seeking a Full-Time Home Health LPN to serve patients in Snohomish County. This role involves building relationships and providing skilled nursing care in patients' homes, including assessments, treatments, medication administration, and wound care. The LPN will also educate patients and families, collaborate with the care team, and maintain accurate documentation to ensure patients remain safe and comfortable in their homes.
Requirements
- Current Washington LPN License
- Current BLS Certification
- Valid driver's license and insured vehicle
Nice To Haves
- Home Health or Hospice experience preferred, but not required
Responsibilities
- Build meaningful one-on-one relationships while providing skilled nursing care in patients' homes
- Assess patient needs, administer treatments, medications, and wound care services
- Educate and empower patients and families to manage their health with confidence
- Collaborate with physicians, therapists, and care team members to deliver outstanding care
- Document visits accurately and maintain compliance with clinical standards
- Help patients remain safe, independent, and comfortable in their own homes
