About The Position
This Home Health LPN position in Oklahoma City allows you to provide skilled nursing care in patients' homes, working as part of a full care team. You will have the opportunity for one-on-one patient time, which is rare in hospital or facility settings. Interim HealthCare of Oklahoma City is a locally owned, Veteran and nurse owned home health, hospice, and personal care agency that has been serving the community since 1999. Requirements
- Active LPN license in Oklahoma
- CPR certification
- Comfortable working independently and managing your own schedule
Nice To Haves
- Home health experience is a plus, but not required. We welcome LPNs from hospital, facility, or clinic backgrounds who are ready for a change
Responsibilities
- Provide skilled nursing visits to patients in their homes under the direction of the RN case manager
- Administer medications, wound care, and other treatments as ordered
- Monitor patient condition and report changes to the RN and physician promptly
- Educate patients and families on their care plan, medications, and home safety
- Document visits thoroughly and in a timely manner
- Build real relationships with the patients you see week after week
