About The Position
We are a leading provider of home health and hospice services in the Philadelphia, PA area. Our mission is to provide exceptional home health and hospice services to patients who need our help the most. We are seeking talented, passionate individuals to join our team and help our patients live happier and healthier lives…in their homes. This position will cover the North Philly service area.
Requirements
- Active LPN state license
- Valid driver's license
- Two or more years of applicable nursing experience
Nice To Haves
- Bilingual (Spanish) preferred
Responsibilities
- Conduct patient home visits, working collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes.
- Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs.
- Manage emergency situations in a prompt, precise, and professional manner.
- Complete medication administration and treatment needs for all assigned patients.
- Provide nursing care in accordance with applicable regulations and Company policy.
- Prepare and maintain accurate and complete clinical and progress notes on a timely basis.
