About The Position

As a CenterWell Home Health nurse reporting to the Clinical Manager, the Home Health LVN/LPN provides personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. The nurse works with physicians, clinicians, patients, and families to support recovery, independence, and individualized care plans.

The role includes educating patients and families about disease management, treatment options, and self-care; documenting visits, interventions, and outcomes; communicating promptly with patients, physicians, the Clinical Manager, and other care team members about patient needs or changes in condition; and participating in care coordination and discharge planning. This is a patient-facing role subject to TB screening and participation in Humana’s driver safety program.

Requirements

• Hold a current nursing license in the practicing state.
• Have a valid driver's license, auto insurance, and reliable transportation.
• Hold a current CPR certification.

Nice To Haves

• Nursing experience in a Home Health or Hospice setting.
• Two years of experience as an LPN/LVN in a clinical setting.

Responsibilities

• Maintain accurate documentation of patient visits, interventions, and outcomes.
• Communicate promptly with patients, physicians, Clinical Manager(s), and other healthcare team members regarding patient or family needs and changes in status.
• Participate in care coordination activities and discharge planning.
