About The Position

As a CenterWell Home Health nurse reporting to the Clinical Manager, the Home Health LPN provides personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. The role supports patients in regaining independence and recovering through individualized care plans developed in collaboration with patients, families, and the healthcare team.

The nurse educates patients and families, documents care and outcomes, communicates promptly with patients and healthcare colleagues about needs or changes in status, and participates in care coordination and discharge planning. This is a patient-facing position that requires TB screening and participation in Humana's driver safety program. The schedule is 40 hours per week.

Requirements

• Current nursing license in the practicing state.
• Valid driver's license, auto insurance, and reliable transportation.
• Current CPR certification.
• One year of experience as an LPN/LVN in a clinical setting.

Nice To Haves

• Nursing experience in a Home Health or Hospice setting.

Responsibilities

• Provide skilled nursing care to patients in their homes under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and families about disease management, treatment options, and self-care techniques.
• Maintain accurate records of patient visits, interventions, and outcomes.
• Communicate promptly with patients, physicians, clinical managers, and other healthcare team members about patient or family needs and changes in status.
• Participate in care coordination activities and discharge planning.
