About The Position
As a CenterWell Home Health nurse, the Home Health Licensed Practical Nurse provides personalized, one-on-one care to patients in their homes to help them regain independence. Reporting to the Clinical Manager and working under the direction of an RN, the nurse collaborates with physicians, clinicians, patients, and families to manage care plans that support recovery.
Requirements
- Current nursing license in the state where the nurse practices.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.
Nice To Haves
- Nursing experience in a Home Health or Hospice setting.
Responsibilities
- Provide skilled nursing care to patients in their homes under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and families about disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient visits, interventions, and outcomes.
- Promptly communicate with patients, physicians, clinical managers, and other healthcare team members about patient or family needs and changes in status.
- Participate in care coordination activities and discharge planning.
