About The Position
Concierge Home Care is hiring a Home Health Licensed Practical Nurse (LPN). This role offers the opportunity to take control of your schedule while providing meaningful one-on-one care in patients' homes. The coverage area is Tampa / Hillsborough County, with territories designed to minimize drive time through clustered scheduling. The position utilizes a pay-per-visit model with opportunities for quarterly bonuses and mileage reimbursement, aiming for average full-time earnings between $55,000 and $70,000+ annually. The company emphasizes flexibility, autonomy, and technology to streamline documentation, allowing nurses to spend more time with patients.
Requirements
- Active, unencumbered Florida Licensed Practical Nurse (LPN) license
- Valid driver’s license, reliable transportation, and auto insurance
- CPR certification

Responsibilities
- Provide skilled nursing care under the supervision of a Registered Nurse
- Perform patient assessments and assist with care planning
- Administer medications and treatments as ordered
- Educate patients and caregivers on disease management and care plans
- Monitor patient condition and report changes to supervising RN
- Collaborate with physicians and interdisciplinary teams
- Complete documentation using streamlined, AI-assisted tools
