About The Position

Terrace Home Health and Hospice is seeking a full-time Nurse Practitioner or Practitioner Assistant to care for home health and hospice patients in the Boise, Idaho area. The practitioner visits patients wherever they reside, including private residences, assisted living facilities, hospitals, long-term care facilities, and group homes. The role involves conducting face-to-face visits independently while working in tandem with the home health and hospice team. The practitioner provides and coordinates patient care, including assessing needs, developing and implementing plans of care, evaluating outcomes, educating and counseling patients and families, and arranging referrals when appropriate. Requirements

Hold an Idaho Nurse Practitioner or Practitioner Assistant license in good standing.
Have prescriptive authority.
Have reliable transportation.
Be proficient with electronic medical records (EMR).
Be highly organized and detail-oriented.
Have strong verbal and written communication skills.
Work well independently.
Complete documentation in a timely manner.
Enjoy working with geriatric patients.

Nice To Haves

Previous home health or hospice experience is preferred but not required.

Responsibilities

Perform face-to-face encounters with patients.
Collect and assess patient care data.
Develop and implement plans of care.
Manage and evaluate care outcomes.
Provide patients and/or families with health education, counseling, and referrals to healthcare professionals or community resources when appropriate.
Participate in case management.
Order treatments and durable medical equipment as indicated.
Visit patients in their residences or care settings, including private residences, assisted living facilities, hospitals, long-term care facilities, and group homes.
Work independently while coordinating with the home health and hospice team.
