Nurse Practitioner- Hospice
About The Position
BAYADA Hospice is hiring a compassionate and dedicated per diem Nurse Practitioner to join its multidisciplinary health care team in Tucson, Arizona. The role provides palliative care to clients so they can receive the medical care they need while remaining at home. Prior home care, hospice, or palliative care experience is preferred but not required. The Nurse Practitioner makes in-home face-to-face hospice recertification visits in designated geographic territories, monitors clients’ conditions, follows up on and carries out doctors’ orders, documents those orders, and performs client assessments as needed. The position requires travel to assigned cases. Requirements
- Hold a current Nurse Practitioner license in Arizona.
- Have a minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Have solid computer skills.
- Have strong interpersonal skills.
- Be able to travel to cases as assigned.
Nice To Haves
- Prior home care, hospice, or palliative care experience.
- Prior experience with electronic medical records (EMR).
Responsibilities
- Make in-home face-to-face hospice recertification visits to clients in designated geographic territories.
- Monitor clients’ conditions and report changes to the Clinical or Client Services Manager.
- Follow up on, execute, and properly document doctors’ orders.
- Perform client assessments as necessary.
- Travel to assigned cases.
