About The Position
Terrace Home Health and Hospice is seeking a full-time Nurse Practitioner or Practitioner Assistant to serve patients in the Boise, Idaho area. The role provides care and services to home health and hospice patients, with visits wherever patients reside, including private residences, assisted living facilities, hospitals, long-term care facilities, and group homes. The practitioner conducts face-to-face visits, works independently while collaborating with the home health and hospice team, and supports high-quality patient care. The position includes a competitive salary and benefits package.
Requirements
- Idaho Nurse Practitioner or Practitioner Assistant license in good standing.
- Prescriptive authority.
- Reliable transportation.
- Proficiency with electronic medical records (EMR).
- Strong organization and attention to detail.
- Strong verbal and written communication skills.
- Ability to work well independently.
- Timely documentation.
- Enjoyment of working with geriatric patients.
Nice To Haves
- Previous home health or hospice experience is preferred but not required.
Responsibilities
- Perform face-to-face encounters with patients.
- Collect and assess patient care data, develop and implement plans of care, and manage and evaluate care outcomes.
- Provide patients and/or families with health education, counseling, and referrals to healthcare professionals and community resources when appropriate.
- Participate in case management.
- Order treatments and durable medical equipment as indicated.
- Visit patients in their residences and other care settings, including private residences, assisted living facilities, hospitals, long-term care facilities, and group homes.
