About The Position

We are seeking a dependable and compassionate Certified Nursing Assistant (CNA) to provide personal care and supportive services to patients in their homes. The CNA works under the supervision of a Registered Nurse and follows the patient's established plan of care. This position is ideal for a CNA who enjoys working independently in the community while being part of a supportive home health team.

Requirements

Current Texas Certified Nurse Aide (CNA) certification in good standing.
Valid driver's license and reliable transportation.
Current automobile insurance.
Ability to travel within assigned service areas.
Ability to work independently and manage an assigned patient schedule.
Strong communication and documentation skills.
Must successfully complete required background and compliance screenings.

Nice To Haves

Previous home health, hospice, long-term care, or similar patient-care experience preferred.
Current CPR/BLS certification preferred.

Responsibilities

Provide personal care as directed by the patient's plan of care, including bathing, grooming, dressing, and hygiene assistance.
Assist patients with mobility, transfers, positioning, and ambulation as appropriate.
Assist with activities of daily living (ADLs).
Observe and report changes in the patient's condition to the supervising nurse.
Measure and document vital signs when assigned.
Maintain accurate and timely documentation of all visits and services provided.
Follow infection-control, safety, and patient-rights requirements.
Maintain patient confidentiality and comply with HIPAA requirements.
Communicate professionally with patients, families, nurses, and other members of the care team.
Travel to patient homes within the assigned service area.
