About The Position

Transition Home Healthcare is seeking a Certified Nursing Assistant (CNA) to provide home health care in the Northwest service area on a PRN basis. The role contributes to patient care in coordination with the assigned nursing care team and supports patients and their families through care, education, documentation, and communication. Requirements

• Hold a CNA license in the state of Texas.
• Have one year of home health experience.
• Be able to lift up to 50 pounds.
• Be able to work occasional weekends, holidays, and evenings.
• Have strong communication and people skills.

• Experience with HCHB EMR.

Responsibilities

• Provide exceptional care in coordination with the assigned nursing care team.
• Maintain patient records, including changes in doctor’s orders, diet, or personal living activities.
• Assist with patient meals, feeding, and basic activities of daily living.
• Deliver weekly reports to the case manager and review important changes in patient conditions.
• Educate patients and family members.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with healthcare providers and other care team members.
