About The Position

Transition Home Healthcare is seeking a CNA to provide home health care in the Northwest service area on a PRN basis. The role provides patient care in coordination with the assigned nursing care team, supports patients with meals and activities of daily living, maintains records and clinical notes, reports changes in patient conditions, and educates patients and family members. Requirements

• CNA license in the state of Texas.
• One year of home health experience.
• Ability to lift up to 50 pounds.
• Ability to work occasional weekends, holidays, and evenings.
• Strong communication and people skills.

Nice To Haves

• Experience with HCHB EMR; preferred but not required.

Responsibilities

• Provide exceptional care in coordination with the assigned nursing care team.
• Maintain patient records, including changes in doctor’s orders, diet, or personal living activities.
• Help with patient meals, feeding, and basic activities of daily living.
• Deliver weekly reports to the case manager and review important changes in patient conditions.
• Educate patients and family members.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with healthcare providers and other care team members.
