Home Health/Hospice Aide
About The Position
VNA Hospice NWI is seeking a dependable, caring, and patient-focused Home Health Aide (HHA) or Certified Nursing Assistant (CNA) to join our hospice team. We are a compassionate hospice organization dedicated to providing exceptional end-of-life care to patients and support to their families. The HHA/CNA provides personal care and supportive services to hospice patients in their homes under the direction of a Registered Nurse and the interdisciplinary hospice team. The ideal candidate is compassionate, reliable, and committed to enhancing the quality of life for patients facing a life-limiting illness.
Requirements
- Must have certification in the State of Indiana
- Must have and maintain a valid driver's license, and automobile insurance.
- Must maintain current CPR certification.
- Must be computer literate.
- High school or equivalent (Required)
- Indiana CNA or HHA (Required)
Nice To Haves
- Prefer minimum of 6 months as a CNA or HHA.
- Current CNAs will receive training and certification as a HHA upon hire.
- BLS Certification (Preferred)
- 1 year experience as CNA or HHA (Preferred)
Responsibilities
- Provide personal care including bathing, grooming, dressing, oral hygiene, and toileting assistance
- Observe and report changes in patient condition to the RN Case Manager
- Maintain accurate documentation of patient visits and care provided
- Assist with light housekeeping related to patient care needs
- Support patients and families with compassion, dignity, and respect
- Follow the established hospice plan of care
- Participate in a rotating on-call schedule to assist with patient needs after hours, weekends, and holidays as assigned
