About The Position

VNA Hospice NWI is seeking Hospice Aides (CNA) or (HHA) for a Full-Time position. This role involves providing personal care to hospice patients at the VNA Hospice Center and in their homes. The ideal candidate is compassionate, reliable, and committed to enhancing the quality of life for patients facing a life-limiting illness. The HHA/CNA provides personal care and supportive services to hospice patients in their homes under the direction of a Registered Nurse and the interdisciplinary hospice team.

Requirements

• Prefer minimum of 6 months as a CNA or HHA.
• Current CNAs will receive training and certification as a HHA upon hire.
• Must have certification in the State of Indiana
• Must have and maintain a valid driver's license, and automobile insurance.
• Must maintain current CPR certification.
• Must be computer literate.
• High school or equivalent (Required)
• Indiana CNA or HHA (Required)
• BLS Certification (Preferred)

Nice To Haves

• CNA or HHA: 1 year (Preferred)

Responsibilities

• Provide personal care including bathing, grooming, dressing, oral hygiene, and toileting assistance
• Observe and report changes in patient condition to the RN Case Manager
• Maintain accurate documentation of patient visits and care provided
• Assist with light housekeeping related to patient care needs
• Support patients and families with compassion, dignity, and respect
• Follow the established hospice plan of care
• Participate in a rotating on-call schedule to assist with patient needs after hours, weekends, and holidays as assigned
