About The Position
The Certified Home Health / Hospice Aide (CHHA) is a paraprofessional member of the home health and hospice care team who works under the supervision of a Registered Nurse or Therapist. The CHHA provides personal care services to patients in their homes to meet individual needs, promote comfort, and support quality of life.
Requirements
- Ability to work effectively with a diverse patient population
- Strong interpersonal and communication skills (oral and written)
- Patient and family sensitivity with a non-judgmental, empathetic approach
- Certified Home Health Aide (CHHA) – State of California, in good standing
- Current BLS certification
- Registration on the Home Care Aide Registry (CA Department of Social Services – Home Care Services Bureau), in good standing
- Valid California driver’s license
- Reliable automobile insured per state and organizational requirements and in good working order
Responsibilities
- Assist patients with personal care needs to promote comfort and safety
- Observe patient condition and promptly report changes to the supervising clinician
- Accurately document care provided and patient observations
