About The Position

The Home Health Hospice Aide participates in delivering high-quality patient care in home or inpatient settings under the supervision of a Franciscan Health Licensed Clinician. The role supports patients with personal care and activities of daily living, documents care, monitors and reports relevant patient information, and coordinates visit schedules with patients or caregivers. The position is based in Lafayette, Indiana, and requires travel up to 75%. Shifts are Monday through Friday, 8 a.m. to 5 p.m.

Requirements

High school diploma or GED.
State licensing board certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA).
American Heart Association CPR certification.
Auto insurance.
Driver's license.

Nice To Haves

One year of experience in a long-term care setting.
One year of home healthcare experience.
One year of hospice experience.

Responsibilities

Assist patients with ambulation and transfers using proper body mechanics.
Communicate changes in patient condition or lack of progress.
Complete required documentation of care provided.
Record intake, output, and vital signs as directed, and communicate care as appropriate.
Assist with personal hygiene and elimination needs according to the established plan of care.
Schedule visits with patients or caregivers.
Assist patients with care and activities of daily living as directed in the established Plan of Care.
