About The Position

The Home Health Hospice Aide participates in delivering high-quality patient care in a home or inpatient setting under the supervision of a Franciscan Health Licensed Clinician. The role includes supporting patients with personal care and activities of daily living, observing and communicating changes in condition, documenting care, and coordinating visits with patients or caregivers. Travel of up to 75% is required. Shifts are Monday through Friday, 8 a.m. to 5 p.m.

Requirements

• High school diploma or GED.
• Current CNA or HHA credential from a state licensing board.
• American Heart Association CPR certification.
• Auto insurance.
• Driver's license.

Nice To Haves

• One year of experience in a long-term care setting.
• One year of home healthcare experience.
• One year of hospice experience.

Responsibilities

• Assist patients with ambulation and transfers using proper body mechanics.
• Communicate changes in patient condition or lack of progress.
• Complete necessary documentation of care provided.
• Record intake, output, and vital signs as directed and communicate care as appropriate.
• Assist with personal hygiene and elimination needs according to the established plan of care.
• Schedule visits with the patient or caregiver.
• Assist patients with care and activities of daily living as directed in the established Plan of Care.
