About The Position

Home Care Network, Inc. is seeking an experienced, passionate, and professional Home Health DSP (with active DoDD Certification) or Home Health Aide (incl. HHA, STNA, CNA, PCA) to join our home health agency and provide daily care, skill development, health monitoring, and advocacy to and on behalf of clients in the greater Toledo, OH area and throughout/adjacent to Lucas County. This is a long-term employment opportunity (2+ years at HCN) including Part-Time (8-31 hrs/wk) or Full-Time (32-40 hrs/wk) that can be built to fit your availability and preferences. HCN provides caregivers with consistent, predictable weekly schedules, matching you to clients with set days/times of care each week that align with your availability and preferences. This provides more stable hours, dependable income, and better work/life balance, and allows deeper relationships, consistency, and trust to be built between you and your clients (and their families). If joining us as a second job, your other employer must be flexible enough to work around your set schedule with us, as we require consistency and reliability for our clients.

Requirements

  • Active DSP Certification from Ohio DODD + 1 or more years of experience caring for persons with disabilities.
  • Either Active CNA or STNA certification + 1 or more years of paid experience Or +2 or more years of paid home health experience for HHA or PCA without certification.
  • Willingness to travel to clients up to a 30-minute radius (from your home) is required.
  • Minimum of 1 year paid in-home care or caregiving experience
  • Actively-certified DSPs, STNAs and CNAs w/ +1 year experience are highly desired (or HHA/PCA with 2+ years paid experience)
  • DSPs also require a valid DoDD DSP certificate within the past 12 months and familiarity with Ohio Board of DoDD rules.
  • Successful completion of all HCN skills tests and competency evaluations.
  • Willingness and timely compliance to maintain all ongoing Training & Certification Requirements
  • Active CPR + FA certification (+ proof/documentation of both readily available)
  • 18yo + active Driver's License / State ID
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius.
  • Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid).
  • Consent + Ability to pass a State & Federal background check (including fingerprints)
  • Willing and able to regularly perform all physical demands of home care (including lifting/carrying up to 30–50 lbs. occasionally).
  • Ability to build trust with individuals; Strong communication skills (verbal & written); Patience, empathy, and compassion.
  • Ability to implement behavior support plans; De-escalation and problem-solving skills; Ability to promote independence rather than dependence.
  • Recognize changes in health status; Follow care plans and medical protocols; Understand and apply infection control and safety procedures.
  • Ability to complete daily service documentation, incident reports, and EVV (Electronic Visit Verification) entries.
  • Possess above-basic computer and mobile device proficiency.
  • Must adhere to Individual rights protections (Ohio Bill of Rights for individuals with DD), Confidentiality (HIPAA compliance), and Mandatory reporting laws (abuse, neglect, exploitation).
  • Must maintain professionalism with individuals, families, and care teams.
  • Successful completion of a formal certification training program and/or a written skills test and competency evaluation.
  • Meet the training and/or certification requirements of the state in which the home health aide services are being provided.
  • Current driver's license, good driving record, and/or reliable transportation is required.
  • Demonstrated ability to read, write, and follow a written Plan of Care.
  • Good interpersonal skills and a passion for caring for those in need.
  • Willingness and aptitude to perform light housekeeping in addition to patient care services as needed.
  • Frequent standing, walking, stopping/bending, infrequent squatting/kneeling, and balancing
  • Weight handled: push, force required: 1-20 lbs. frequently, 20-30 lbs. occasionally, 30-50 lbs. infrequently, carry/lift:1-20 lbs. frequently, 20-30 lbs. occasionally, 30-50 lbs. infrequently.

Nice To Haves

  • Additional Medication Certifications, Hoyer lift experience, specific disease-state care experience, etc., are highly desirable but not required.
  • Up to 45- or 60-minute client travel radius (from your home) is highly desired.
  • Medication Administration Certification is highly preferred
  • Experience with modern EMR/EHRs is a plus.
  • 3-5 years paid caregiving experience with exceptional referrals
  • Experience with: G-tubes / feeding support
  • Seizure protocols
  • Behavioral health or crisis response
  • EVV activities and compliance
  • Familiarity with Medicaid waiver services
  • CHHA, CNA, and STNA accreditation desirable but not required.

Responsibilities

  • Assists in the patient in taking medications under the direction and supervision of the Registered Nurse. This includes opening a container or pill caddy & handing it to the patient, but does not include administration of the medications.
  • Knows and follows infection control policies, including proper hand washing techniques.
  • Reports any observed or reported changes in the client's condition and/or needs to the Registered Nurse.
  • Documents care provided and completes the forms required for the client's records.
  • Completes the appropriate records to document cares given and pertinent observations.
  • Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager.
  • Demonstrates safe practice in the use of equipment.
  • Notifies supervisor of educational needs.
  • Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.
