About The Position
Sentara Home Care is seeking to hire a qualified individual to join our team as a Home Health Coordinator- RN. The RN Home Health Coordinator coordinates and supervises all nursing care for participants according to program standards. Includes interaction with other facility staff. Actively participates in interdisciplinary team meetings. Conducts initial and periodic assessments. Administer nursing care to ill, injured, convalescent or disabled participants. Advise participants on health maintenance and disease prevention. Administers medication if needed.
Requirements
- Associate's Degree Nursing (Required)
- RN (Required) - Active in VA
- BLS (Required within 90 days)
- 1-year minimum nursing experience (required)

- Home Health experience (highly preferred)
