Home Health Coordinator- Registered Nurse
About The Position
Sentara Home Care is seeking to hire a qualified individual to join our team as a Home Health Coordinator- RN. This is a Full Time position with a Day Shift schedule, located in Suffolk, VA. Mileage reimbursement will be paid for all miles driven between patient visits. We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The RN Home Health Coordinator coordinates and supervises all nursing care for participants according to program standards. This role includes interaction with other facility staff and actively participates in interdisciplinary team meetings. The coordinator conducts initial and periodic assessments, administers nursing care to ill, injured, convalescent or disabled participants, and advises participants on health maintenance and disease prevention. Medication administration may also be required.
Requirements
- Associate's Degree Nursing (Required)
- RN (Required) - Active in VA
- BLS (Required within 90 days)
- 1-year minimum nursing experience (required)
