Home Health Coordinator – Registered Nurse
About The Position
Sentara Home Care is seeking to hire a qualified individual to join our team as a Home Health Coordinator- RN. This position is full-time, day shift, located in Virginia Beach, VA. Mileage reimbursement will be paid for all miles driven between patient visits. We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The RN Home Health Coordinator coordinates and supervises all nursing care for participants according to program standards. This includes interaction with other facility staff and actively participating in interdisciplinary team meetings. The role involves conducting initial and periodic assessments, administering nursing care to ill, injured, convalescent or disabled participants, advising participants on health maintenance and disease prevention, and administering medication if needed.
Requirements
- Associate's Degree Nursing (Required)
- RN (Required) - Active in VA
- BLS (Required within 90 days)
- 1-year minimum nursing experience (required)

- Home Health experience (highly preferred)
