About The Position

As a Home Health CNA at CenterWell Home Health, you will coordinate with an LPN to pick up and deliver test kits for the STARS program. The territory covers Raleigh, Durham, Rocky Mount, and Goldsboro. You will take vital-sign measurements, report changes in patient condition, document care and interactions, follow infection-control principles, and help maintain a clean, safe, and comfortable environment. This patient-facing role is subject to Humana’s TB screening program and driver safety requirements.

Requirements

Complete a Certified Nursing Assistant program within the last 24 months; CNA license is required.
Meet applicable state certification requirements.
Have a valid driver's license, auto insurance, and reliable transportation.
Be in good standing on the HHA Registry, if applicable, and have completed an HHA/CNA course to work for a Medicare-certified agency.
Have at least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency.
If selected, complete the required TB screening.
Maintain personal vehicle liability insurance meeting the residing state's minimum requirements or the stated higher limits: $25,000 bodily injury per person, $25,000 bodily injury per event, and $10,000 property damage.

Responsibilities

Coordinate with an LPN to pick up and deliver test kits for the STARS program.
Take accurate temperature, pulse, and blood pressure measurements and recognize and report abnormal results to a supervisor.
Report changes and document pertinent patient information and care rendered to support continuity of care.
Document interactions with patients, caregivers, doctors, and other staff members appropriately, legibly, thoroughly, and within the time allowed.
Practice acceptable infection-control principles.
Provide a clean, safe, and comfortable environment.
