About The Position

CenterWell Home Health is hiring a Home Health CNA to support its STARS program across the Raleigh/Durham–Rocky Mount–Goldsboro territory. Reporting to the Associate Director, Branch Administration, the CNA coordinates with an LPN to pick up and deliver test kits. The role also involves taking and documenting patient measurements, reporting changes in patient condition, maintaining infection-control practices, and helping provide a clean, safe, and comfortable environment.

Requirements

• Complete a Certified Nursing Assistant program within the last 24 months.
• A CNA license is required.
• Meet applicable state certification requirements.
• Have a valid driver's license, auto insurance, and reliable transportation.
• Be in good standing on the HHA Registry, if applicable, and have completed an HHA/CNA course to work for a Medicare-certified agency.
• Have at least one year of experience within the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency.
• If selected, complete TB screening as part of the patient-facing role.
• Maintain personal vehicle liability insurance in accordance with state minimum limits or the stated higher limits: $25,000 bodily injury per person, $25,000 bodily injury per event, and $10,000 property damage.

Responsibilities

• Coordinate with an LPN to pick up and deliver test kits for the STARS program.
• Take accurate temperature, pulse, and blood pressure measurements and report abnormal results to a supervisor.
• Report changes and document pertinent patient information and care rendered to support continuity of care.
• Document interactions with patients, caregivers, doctors, and other staff members appropriately, legibly, thoroughly, and within the time allowed.
• Practice acceptable infection-control principles.
• Provide a clean, safe, and comfortable environment.
