Amazing Care Home Health is seeking a full-time RN Clinical Supervisor for our Skilled Nursing Department in our Loveland, CO office. As we expand, we need a dedicated individual to join our team and uphold our commitment to compassion and quality service.

Requirements

  • Current Supervisory/Case Management experience
  • RN License
  • Experience in home health and a management role
  • Ability to work independently and prioritize tasks effectively

Responsibilities

  • Coordinate all aspects of patient care, including recertification, admission, treatment, and discharge planning
  • Provide clinical and clerical management support
  • Collaborate with case managers and clinical field personnel to ensure optimal patient care
  • Work autonomously and demonstrate strong leadership skills
  • Work a flexible schedule, mix of in the office and field work
  • Exhibit motivation and eagerness to continually grow skills, with flexibility to adapt to changes
  • Demonstrate strong communication skills and the ability to navigate obstacles
  • Act as a team player and be willing to commute to patient homes when needed, assisting with visits as required
