Amazing Care Home Health is seeking a full-time RN Clinical Supervisor for its Skilled Nursing Department in the Loveland, CO office. This role is crucial for overseeing the care management for an assigned caseload and clinicians, ensuring the delivery of high-quality care to patients.

Requirements

  • Required Home Health and Oasis Experience
  • Current Supervisory/Case Management experience
  • RN License
  • Experience in home health and a management role
  • Ability to work independently and prioritize tasks effectively

Responsibilities

  • Coordinate all aspects of patient care, including recertification, admission, treatment, and discharge planning
  • Provide clinical and clerical management support
  • Collaborate with case managers and clinical field personnel to ensure optimal patient care
  • Work autonomously and demonstrate strong leadership skills
  • Work a flexible schedule, mix of in the office and field work
  • Exhibit motivation and eagerness to continually grow skills, with flexibility to adapt to changes
  • Demonstrate strong communication skills and the ability to navigate obstacles
  • Act as a team player and be willing to commute to patient homes when needed, assisting with visits as required
