About The Position

Puget Sound Home Health & Hospice, a provider of exceptional care for over 30 years, is expanding and seeking a dedicated Home Health Clinical Manager for its Pierce County division. This role requires a Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) license and experience in a Medicare-certified home health agency (OASIS trained). The employer for this position is The Pennant Group, Inc., a holding company of independent operating subsidiaries that provide healthcare services. Each business is operated by a separate, independent operating subsidiary.

Requirements

• Registered Nurse (RN), Physical Therapist (PT), or Occupational Therapist (OT) license
• Minimum 2 years of healthcare management experience
• At least 1 year of home health/hospice experience with OASIS training
• Current BLS certification
• Washington state license
• Licensed and insured driver

Nice To Haves

• BSN degree preferred
• Experience with Home Care Home Base preferred
• Knowledge of Medicare COPs, PDGM, VBP

Responsibilities

• Ensure compliance with home health/hospice laws and regulations
• Review referrals and manage assignments
• Oversee patient charts and clinical courses
• Assist with staff training, hiring, and mentoring
• Participate in QAPI, OASIS review, and coding workflow
• Some direct patient care and on-call duties required
