About The Position

Identifies, evaluates, and enrolls patients into Mount Sinai at Home programs. This role is on-site and integrated into clinical teams at our health system hospitals, working directly with patients and clinical teams in the ED and hospital floors. Coordination is done telephonically from an office setting. Ensures correct admission orders and home safety/eligibility for the program.

Requirements

Graduation from an accredited Nursing program.
Bachelor’s Degree in Nursing (BSN) or Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year.
Any Associate’s Degree in Nursing (ADN) RN with Nursing experience must be approved by the CNO.
Relevant clinical competence in the area of nursing practice assigned; new graduates must possess current knowledge of the nursing process and its application.
Licensed as a registered nurse with current registration in New York State.
Basic Life Saver (BCLS) certification from AHA.
Current unencumbered license to practice as an RN.
1+ year experience working as an RN.
Ability to work both day and evening shifts, rotating holidays, and be on call.

Nice To Haves

2+ years experience as RN
Experience in the Emergency Departments, Intensive Care Unit or Med Surg.

Responsibilities

Identify and evaluate patients for appropriateness for Mount Sinai at Home programs.
Possess clinical knowledge and experience to identify, present, and discuss potential patients for program inclusion across multiple medical specialties and locations.
Facilitate coordination and communication between all members of the care team to coordinate admissions.
Ensure knowledge and acceptance of Mount Sinai at Home with patients, family members, and ED/hospital teams.
Maintain all required documentation.
Interact with patients and teams to coordinate services ordered for physicians and move care plans forward.
Clearly communicate transition plans and information to all members of the care team.
Build and maintain collaborative professional working relationships with physicians, nurses, and the community.
Screen and question patients and families to ensure safe home environments.
Ensure that patients have access to appropriate services to meet their provider-directed care plan admission needs.
Comply with policies, procedures, and regulatory mandates, including abiding by the terms of MSHS compliance.
Demonstrate a professional, courteous, and respectful attitude with patients, families, and hospital staff.
Serve as the primary point of communication between inpatient teams and Hospital at Home.
Answer telephonic and email correspondences from patients, caregivers, and healthcare providers.
May be required to perform other duties as assigned or perform other duties when necessary.
