About The Position

Cleveland Clinic Home Care provides skilled nursing, social services, and physical, occupational, and speech therapy for patients returning home after a hospital stay, outpatient procedures, or doctor's visit. Our home care team continues Cleveland Clinic's treatment at home, providing short-term intermittent acute care. Our nurses work autonomously, meeting patients where they live, which requires strong assessment skills and critical thinking. We provide flexibility to the nurse and the patient when coordinating care within a team environment. As an RN, you will educate and follow the patient through a short-term intermittent recovery process. This role supports the Eastern Cuyahoga county, including cities like East Cleveland, Cleveland Heights, South Euclid, Lyndhurst, and others as needed. Cleveland Clinic Home Care provides a generous, in-depth orientation tailored to each individual and their experience level. This position is eligible for student loan repayment. Caregivers in this role will work from 9:00 am to 5:00 pm, including one weekend a month and two holidays a year.

Requirements

  • Graduate from an accredited school of Professional Nursing
  • Current state licensure as a Registered Nurse (RN)
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA)
  • Valid Ohio Driver's (VDL) license upon hire and per policy 8-175 thereafter
  • Proof of 100,000/300,000 automobile insurance coverage upon hire and per policy 8-175 thereafter
  • One year of current experience as an RN
  • One year of experience in an acute Medical/Surgical or Home Care setting
  • Requires full range of motion, manual and finger dexterity and eye-hand coordination.
  • Requires standing and walking for extensive periods of time.
  • Requires corrected hearing and vision to normal range.
  • Requires some exposure to communicable disease or bodily fluids.
  • Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
  • Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.
  • Must be able to read, write and communicate with patients.
  • Follows standard precautions using personal protective equipment as required.

Nice To Haves

  • Bachelor’s of Science in Nursing (BSN)
  • Familiarity with complex medical treatments
  • Experience working in homecare, med/surg, ICU and/or ED areas

Responsibilities

  • Educate and follow the patient through a short-term intermittent recovery process.
  • Perform lab draws, including peripheral sticks or central line draws.
  • Provide wound care for all types of wounds, including measurements, documentation, and wound vac experience.
  • Administer IV medications, including IV insertion, IV antibiotics, IV push medications, and central line experience.
  • Manage own patient assignment and make critical decisions in the field.
  • Treat patients in their home environment, allowing for personalized care.
  • Gain exposure to a wide range of patient conditions.
  • Use mobile documentation and a wide range of technologies to deliver efficient, high-quality care in the home setting.
