Home Health Care Admission RN - Greater Nashua
About The Position
This position involves admission to HHHC's homecare division. The RN will complete an initial home health assessment of the patient and family to determine home care needs. They will provide a complete physical assessment and history of current and previous illness(es), utilizing the nursing process in planning, providing, and evaluating skilled needs. The role requires completing a comprehensive patient assessment reflected in accurate OASIS scoring and developing a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. The patient and family will be included in the planning and revising process.
Requirements
- Graduate of an accredited school of nursing.
- Associates degree in nursing.
- Registered as RN with New Hampshire’s Board of Nursing.
- A minimum of one (1) year recent acute care experience.
- Ability to exercise initiative and independent judgment.
- Excellent verbal and written communication skills and competent in problem solving abilities.
