Home Health Care Admission RN - Greater Londonderry
About The Position
This role involves admission to HHHC's homecare division. The Registered Nurse (RN) will complete an initial home health assessment of the patient and family to determine home care needs. The RN will provide a complete physical assessment and history of current and previous illness(es), utilizing the nursing process in planning, providing, and evaluating skilled needs. A comprehensive patient assessment that is reflected in accurate OASIS scoring will be completed. The RN will develop a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions, including the patient and family in the planning and revising process.
Requirements
- Graduate of an accredited school of nursing.
- Associates degree in nursing.
- Registered as RN with New Hampshire’s Board of Nursing.
- A minimum of one (1) year recent acute care experience.
- Ability to exercise initiative and independent judgment.
- Excellent verbal and written communication skills and competent in problem solving abilities.
