About The Position

Choice Health at Home is seeking an energetic, relationship-driven, and motivated Home Health Business Development Liaison to help grow our presence within the community and expand access to exceptional home health services. In this role, you will serve as a key ambassador for Choice Health at Home by connecting with physicians, hospitals, case managers, skilled nursing facilities, assisted living communities, and other healthcare professionals to promote our patient-centered care services.

Requirements

Minimum of 2 years of home health or hospice sales experience preferred
Strong relationship-building and communication skills
Self-motivated with the ability to work independently
CRM software experience preferred
HomeCare HomeBase experience preferred
Excellent presentation and organizational skills
Valid Driver’s License
Current Auto Insurance
Reliable transportation

Responsibilities

Build and maintain strong referral relationships with healthcare providers and community partners
Promote Choice Health at Home’s services and position the agency as the preferred home health provider
Identify and develop new business opportunities and referral sources
Collaborate with clinical and operational leadership to support agency growth and patient care initiatives
Support program development based on community and market needs
Maintain compliance with all local, state, and federal healthcare marketing regulations
Partner with interdisciplinary teams to ensure excellent patient and family experiences
Represent the agency at community events, healthcare meetings, and networking opportunities
Track referral activity and maintain accurate CRM documentation
Meet and exceed monthly admission and growth goals
