Requirements
- Graduate of an accredited school of nursing and active state RN license.
- Minimum of 1 year of recent (within the last 3 years) RN psychiatric experience.
- Valid driver’s license and auto insurance.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
Nice To Haves
- Bachelor’s degree with 1 year of home health care experience strongly preferred.
- Homecare Homebase (EMR) experience strongly preferred.
Responsibilities
- Conduct initial and ongoing comprehensive assessments to determine home care needs, including physical and behavioral health evaluations.
- Develop, implement and adjust care plans based on patient status and nursing diagnosis.
- Administer prescribed medications and treatments, provide therapeutic and rehabilitative nursing interventions, and educate patients and families on health management, preventive care, and recovery strategies.
- Collaborate with physicians, specialists, and community healthcare professionals to ensure seamless, holistic patient care.
- Communicate updates regarding patient condition and secure necessary physician orders.
- Identify discharge planning needs and coordinate necessary follow-ups to promote patient well-being.
- Prepare and maintain accurate clinical records, ensuring timely updates for physicians and compliance with healthcare regulations.
- Review patient status and modify care documentation, providing consistent evaluations and care revisions as needed.
- Participate in on-call duties as required, ensuring patients receive timely support and intervention.
- Coordinate necessary medical equipment and services to enhance patient comfort and recovery.
