About The Position

We are seeking a highly-skilled, compassionate, and professional Home Health Aide (STNA or CNA highly desired | experienced HHA, CHHA, DSP, or PCA will also be considered) to join our home health agency and provide hands-on personal care and support to clients in Magnolia, OH and surrounding areas of Stark County. A successful DSP provides personal care services under the direction of the Registered Nurse or Therapist in their region. You will be assigned to specific clients by the Registered Nurse or other appropriate professionals and perform services for clients as necessary to maintain personal comfort and safety in their home.

Requirements

  • Minimum of 1 year paid in-home care or caregiving experience (STNA/CNA certification in lieu of experience will be considered)
  • Successful completion of all agency-provided HHA/PCA orientation, skills tests and competency evaluations.
  • Willingness and timely compliance to maintain all ongoing Training & Certification Requirements
  • Active CPR + FA certification and documentation of both (+ proof/documentation of both readily available)
  • Ability to build trust with individuals with developmental disabilities
  • Strong communication skills (verbal & written)
  • Patience, empathy, and compassion.
  • Ability to implement behavior support plans
  • De-escalation and problem-solving skills
  • Ability to promote independence rather than dependence.
  • Recognize changes in health status
  • Follow care plans and medical protocols
  • Understand and apply infection control and safety procedures.
  • Ability to complete daily service documentation, incident reports, and EVV (Electronic Visit Verification) entries.
  • Possess above-basic computer and mobile device proficiency.
  • Must adhere to Individual rights protections (Ohio Bill of Rights for individuals with DD), Confidentiality (HIPAA compliance), and Mandatory reporting laws (abuse, neglect, exploitation)
  • Must maintain professionalism with individuals, families, and care teams
  • Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid)
  • Character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful
  • Willingness and ability to perform required physical demands (see below)
  • Consent + Ability to pass a State & Federal background check (including fingerprints)
  • Must be eligible to work in home and community-based settings (HCBS)
  • 18yo + active Driver's License / State ID + US Work Authorization
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients

Nice To Haves

  • Hoyer Lift experience (minimum of 1 year; 2+ desired)
  • Medication Administration Certification is highly preferred
  • Experience with modern EMR/EHRs is a plus.
  • G-tubes / feeding support
  • Seizure protocols
  • Behavioral health or crisis response
  • High School diploma or GED
  • Active STNA (State Tested Nurse Aide) or CNA certification, or previous home health aide certification
  • Previous home health experience (1-3 years)
  • Familiarity with Medicaid waiver services
  • Up to 45- or 60-minute client travel radius (from your home) is highly desired

Responsibilities

  • Personal Care & Activities of Daily Living (ADLs)
  • Safe transfers including Hoyer Lift (minimum of 1 year experience; 2+ desired) as well as mobility and positioning support
  • Support safe use of assistive devices (walkers, wheelchairs, lifts)
  • Nutrition & Meal Support
  • Homemaking & Environmental Support (IADLs) including light housekeeping, errands, and tasks required to maintain a clean, safe, and organized home environment
  • Community Engagement & Appointment Support, Coordination, and (in approved situations) Transportation
  • Basic Health and Safety Monitoring
  • Care Plan Implementation
  • Dementia & Behavioral Support (if applicable)
  • Pro-active observation and excellent electronic documentation
  • Client Rights, Dignity & Confidentiality
  • Total Care Team Collaboration
  • Ongoing Training & Certification Requirements
  • Schedule Options Primary: Every other Saturday and Sunday (EO Weekend) Requires 4-8 hour shifts per day [8-16 hrs/wk]
  • Additional Weekday or alternate Weekend shifts with additional clients may be available if additional hours are desired
  • Part-Time minimum schedule of 16 hours/wk required
  • Local travel within Stark County | Expanded cases/hours may include adjacent Counties (not required)
