About The Position

LifeCircles PACE is seeking a dedicated and compassionate Home Health Aide (HHA) to provide in-home care and support that helps older adults remain safely in their homes while maintaining dignity and independence. The role is based in Muskegon, Michigan, in the Home Care department. The schedule is 40 hours per week, Monday through Friday, from 8:00 a.m. to 4:30 p.m., with holiday rotation.

The aide assists participants with personal care according to their Life Plan, household tasks, meals and hydration, mobility, vital checks, and maintaining a comfortable living space. The aide responds compassionately, observes and reports changes in participants’ conditions to the interdisciplinary team and leadership, and maintains confidentiality and professionalism.

Requirements

• Certified Nurse Aide (CNA) per state requirements.
• Reliable transportation and a valid driver’s license.
• High school diploma or GED.
• Excellent communication and observation skills.

• Previous experience providing care to older adults.

Responsibilities

• Assist with personal care, including bathing, grooming, dressing, and toileting, according to the participant’s Life Plan.
• Support housekeeping tasks such as laundry, sweeping, and grocery shopping.
• Help participants with meals and hydration.
• Support mobility, including turning, positioning, and safe transfers.
• Measure and record vital signs, weight, and intake and output.
• Keep rooms tidy, comfortable, and stocked with essentials.
• Respond to participants with kindness and compassion.
• Observe and promptly report changes in participants’ conditions to the interdisciplinary team and leadership.
• Maintain confidentiality and professionalism while providing care.
