About The Position

LifeCircles PACE is seeking a dedicated and compassionate Home Health Aide (HHA) to provide in-home care and support to older adults, helping them remain safely in their homes while maintaining dignity and independence. The role is in the Home Care department of LifeCircles PACE in Holland, Michigan.

The schedule is 40 hours per week, Monday through Friday, with a holiday rotation, from 8:00 a.m. to 4:30 p.m. Duties include personal care, household support, mealtime assistance, mobility support, vital-sign checks, and reporting changes in participants’ conditions to the interdisciplinary team and leadership. The listed duties are a summary and are not necessarily the only duties required.

Requirements

• Certified Nurse Aide (CNA) per state requirements.
• Reliable transportation and a valid driver’s license.
• High school diploma or GED.
• Excellent communication and observation skills.

• Previous experience providing care to older adults.

Responsibilities

• Assist participants with personal care activities of daily living, including bathing, grooming, dressing, and toileting, following each participant’s Life Plan.
• Assist with household tasks such as laundry, sweeping, and grocery shopping.
• Help participants with meals and hydration.
• Support mobility, including turning, positioning, and safe transfers.
• Measure and record vital signs, weight, and intake and output.
• Keep participants’ rooms tidy, comfortable, and stocked with essentials.
• Respond to participants with kindness and compassion.
• Report changes in participants’ conditions promptly to the interdisciplinary team and leadership.
• Maintain confidentiality and demonstrate respect and professionalism.
