About The Position

The Home Health Aide (HHA) provides essential personal care and supportive services under the supervision of the RN Case Manager or designated administrator. The HHA works closely with clients and families to promote safety, comfort, and independence while strictly adhering to the established Plan of Care (POC).

Requirements

  • High school diploma or GED
  • Successful completion of an approved HHA training program
  • Completion of required competency testing
  • Current CPR/First Aid certification
  • TB screening within the past year
  • Clear state background check
  • Reliable transportation with valid auto insurance
  • Ability to lift at least 25 pounds
  • Ability to read, write, and communicate effectively in English
  • Strong communication skills and ability to follow written and verbal instructions

Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, oral care, toileting, and transfers/ambulation.
  • Provide meal planning, preparation, and feeding support according to dietary guidelines.
  • Empty catheter and colostomy bags and accurately document output.
  • Change bed linens and assist with laundering soiled clothing.
  • Perform light housekeeping to maintain a clean, safe, and healthy environment.
  • Accompany clients to appointments and errands, providing transportation and companionship.
  • Engage clients in recreational and social activities.
  • Provide emotional support to clients and families.
  • Accurately document client care, condition, progress, and concerns on the same day services are provided.
  • Record vital signs, meal consumption, fluid intake, and urine/bowel output as directed.
  • Promptly report any changes in client condition to the RN Case Manager or administrator.
  • Immediately report any client hospitalization.
  • Follow the HHA Plan of Care at all times.
  • Maintain infection control standards and safe home practices.
  • Recognize emergencies and follow agency protocols.
  • Protect client confidentiality and uphold ethical standards.
  • Practice strictly within the HHA scope of practice.
  • Respect clients’ Bill of Rights.
  • Comply with the company’s zero-tolerance drug and alcohol policy.
  • Participate in weekly training or continuing education sessions.
