About The Position

Home Care Network (HCN) is seeking a Home Health Aide (HHA) to provide personal care services under the direction of a Registered Nurse or Therapist. This role serves clients in Coshocton, Ohio, and surrounding areas. As an HHA, you will deliver direct patient care, measure vital signs, assist with personal care tasks and activities of daily living, perform household tasks essential to healthcare, and assist patients with medications under supervision. You will also identify and report any abnormal findings or changes in the patient's condition and assist with therapy exercises. This is a PRN (as needed) position with no guaranteed hours, offering flexibility for individuals seeking supplemental income or a variable schedule. Requirements

  • Active CNA or STNA certification + 2 or more years of paid experience
  • Or Active DSP Certification from Ohio DODD + 2 or more years of experience caring for persons with disabilities AND willingness to care for non-DODD clients.
  • Or +3 or more years of paid home health experience for HHA or PCA without certification.
  • Successful completion of HCN's written skills test and/or competency evaluation.
  • Meet/Completion of all training and/or certifications required by the state in which the home health aide services are being provided.
  • CPR and First Aide Certifications (active/current certs preferred but may also be obtained prior to hire date through HCN or accredited third party).
  • Ability to perform all typical physical tasks on a regular basis, and willingness to do so safely, including: Frequent: standing, walking, stopping, ending; Infrequent: squatting/kneeling, balancing; Pushing and handling weight/force: 1-20 lbs. frequently, 20-30 lbs. occasionally, and 30-50 lbs. infrequently; Lifting and carrying: 1-20 lbs. frequently, 20-30 lbs. occasionally, 30-50 lbs. infrequently.
  • Willingness and aptitude to perform light housekeeping in addition to patient care services as needed.
  • Strong track record of attendance, dependability, and employment retention with previous employers.
  • Good interpersonal skills and compassion for helping others live their best lives.
  • Demonstrated ability to read, write, and follow a written Plan of Care, incorporating attention to detail in following instructions, documenting notes, and communicating with our agency.
  • Professional character traits: reliable, honest beyond reproach, punctual, articulate, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, self-driven, problem-solver, compliant, and confidential.
  • Be at least eighteen (18) years of age.
  • Valid driver's license/ID and reliable transportation to/from client care locations.
  • Consent and ability to pass a State and/or Federal Background Check.
  • Must have a positive attitude, strong caregiving skills, and a compassionate mindset.
  • Must remain calm, respectful, dependable, emotionally mature, and reassuring, builds trust quickly, communicates professionally, and provides excellent care in challenging situations.
  • Must be comfortable working in a variety of home environments, including homes with pets, smoke odors, clutter, family members or roommates present, and other common residential conditions.
  • Must maintain flexibility to accept assignments based on client and business needs within their stated availability.
  • Must complete all required documentation accurately, thoroughly, and on time.

Nice To Haves

  • Additional Medication Certifications, Hoyer lift experience, specific disease-state care experience, etc., are highly desirable but not required.
  • Up to 45- or 60-minute client travel radius (from your home) is highly desired.
  • 3-5 years (or more) of paid experience in a home health or caregiving setting.
  • Paid experience with a Home Health Agency.
  • Formal certification or training program: STNA (or 2026 graduation date) highly preferred; CNA or CHHA still encouraged to apply.
  • Specialized certification or training program, such as DODD-DSP, Med Certification, EMR/EHR, etc.
  • EMR/EHR experience and proficiency (ContinuLink highly desirable).

Responsibilities

  • Deliver direct patient care and support, following the established plan of care, to pediatric, adult, and geriatric patients.
  • Measure and record vital signs and other specified metrics as instructed.
  • Assist with personal care tasks and activities of daily living (e.g., bathing, oral hygiene, nail & skin care, shaving, dressing).
  • Perform household tasks essential to health care and living at home (e.g., meal preparation/feeding, laundry, light housekeeping).
  • Assists patients in taking medications under the direction and supervision of the Registered Nurse. This includes opening a container or pill caddy & handing it to the patient but does not include administration of the medications.
  • Identify and report any abnormal findings or changes in the patient's condition.
  • Help with therapy exercises under supervision of our therapists (e.g., range of motion exercises, ambulation assistance).
  • Complete all required documentation accurately, thoroughly, and on time, including care notes, visit records, EVV requirements, incident reporting, and other client-related documentation.
