About The Position

Custom Home Health is hiring a full-time Home Health Aide to provide hands-on personal care that supports patients’ comfort, dignity, and well-being. The role includes assisting with personal hygiene and daily activities, maintaining patients’ living environments, and helping with meals. The aide follows individualized care plans in collaboration with nurses and interdisciplinary teams, observes and reports changes in patients’ conditions, and documents care according to organizational standards. The position also involves participating in orientation, staff meetings, and training programs, including QAPI and Emergency Preparedness. Other duties may be assigned.

Requirements

Valid state certification.
At least one year of certified nursing experience.
At least 18 years old.
Ability to read and follow written instructions and accurately document care.
Current CPR certification.
Demonstrates tact, patience, excellent hygiene, and a commitment to professional care.

Responsibilities

Assist patients with personal hygiene, including bathing, oral care, and grooming.
Support daily activities such as dressing, toileting, and ambulation.
Maintain a clean and orderly living environment for patients.
Prepare and serve nutritious meals and assist with feeding when needed.
Follow personalized care plans in collaboration with nurses and interdisciplinary teams.
Observe and report significant changes in a patient’s condition to the assigned nurse.
Document care in accordance with organizational standards and maintain professional conduct.
Participate in orientation, staff meetings, and training programs, including QAPI and Emergency Preparedness.
