About The Position

We are looking for a reliable team member who enjoys starting their day early, with morning shifts beginning between 6:00 and 7:00 a.m. This role requires someone who is comfortable providing hands-on support and assisting with ADLs, ensuring clients receive consistent, high-quality care to begin their day. Requirements

A compassionate, patient, and caring nature.
Strong interpersonal and communication skills.
Experience providing care to clients in a hands-on environment (Home Health, Assisted Living, Group Home, Hospital Setting)
Reliable vehicle and a valid driver’s license with Auto Insurance.
Negative TB test or Cleared Chest-Xray.
Availability to work 2 weekend days per month.

Responsibilities

Provide companionship and conversation to enhance the lives of our clients.
Assist with daily activities such as meal preparation, light housekeeping, and errands.
Provide physical assistance such as bathing, dressing, grooming, transferring, and ambulation.
Accompany clients to appointments or social outings, providing transportation as needed.
Monitor and report changes in the client’s health, behavior, and needs.
Act as a member of the Care coordination team to communication with the RN Case Manager to update the plan of care.
