Requirements
- Current HHA or CNA certification, as applicable.
- Previous experience providing in-home or direct patient care preferred.
- Compassionate, dependable, and professional demeanor.
- Strong communication and interpersonal skills.
- Ability to work independently and follow a patient-specific plan of care.
- Reliable transportation and the ability to consistently work the assigned schedule.
- Ability to pass any required background checks and meet applicable employment or contracting requirements.
- Ability to monitor vital signs
- Current CPR certification

Responsibilities
- Provide compassionate, one-on-one care to the patient in the comfort and familiarity of their home.
- Assist with activities of daily living (ADLs), including personal hygiene, grooming, dressing, mobility, and other personal care needs.
- Provide light housekeeping to help maintain a safe and comfortable home environment.
- Prepare meals and assist with meal-related needs as appropriate.
- Monitor and document vital signs, including temperature, respiration, and other measurements as directed by the plan of care.
- Provide medication reminders in accordance with the established plan of care and within the scope of the caregiver’s role.
- Maintain accurate and timely records and documentation of services provided and the patient’s condition.
- Communicate appropriately with the patient’s family and other authorized individuals regarding the patient’s care, needs, and well-being.
- Follow the established care plan and report changes in the patient’s condition or care needs to the appropriate person.
- Protect the patient’s privacy, dignity, and confidentiality at all times.
