Requirements
- Current HHA or CNA certification, as applicable.
- Previous experience providing in-home or personal care is preferred.
- Compassionate, reliable, and patient-centered approach to care.
- Strong communication and organizational skills.
- Ability to work independently while following the established plan of care.
- Dependable transportation and the ability to consistently arrive as scheduled.
- Ability to monitor vital signs
- Current CPR certification
Nice To Haves
- Previous experience providing in-home or personal care is preferred.
Responsibilities
- Provide compassionate, one-on-one care in the patient’s home.
- Assist the patient with activities of daily living (ADLs) and personal care.
- Provide assistance with mobility and other routine daily activities as needed.
- Prepare meals and assist with maintaining a clean and safe home environment.
- Perform light housekeeping related to the patient’s care.
- Monitor and document vital signs, including temperature, respiration, and other measurements as directed by the plan of care.
- Provide medication reminders in accordance with the established plan of care and applicable regulations.
- Maintain accurate and timely records and documentation of services provided.
- Communicate professionally and respectfully with the patient’s family and other authorized individuals regarding the patient’s care and needs.
- Follow the physician-established plan of care and report any significant changes in the patient’s condition to the appropriate individual.
