About The Position
Community Home Health Care is seeking compassionate and dedicated Home Health Aides / Certified Nursing Assistants (CNA) to join our team in Fort Wayne, IN. In this role, you will provide essential in-home care and support to individuals who require assistance with daily living activities, promoting their independence and enhancing their quality of life. We have immediate openings for individuals who are dependable and comfortable working: Early mornings, Late evenings, Weekends, Rotating weekend schedules. These shifts support some of our most vulnerable clients, and we are looking for team members who care deeply about consistency and strong client relationships.
Requirements
- Valid CNA certification or Home Health Aide training.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Reliable transportation to travel to client homes in Fort Wayne, IN.
- Compassionate, patient, and attentive to the needs of clients.
- Background check and drug screening may be required.
- Active HHA/CNA certification (or willingness to complete required HHA training).
- Strong work ethic and passion for serving the community.
- Reliable transportation and ability to meet shift expectations.
- Respectful communication and a commitment to high-quality care.
Responsibilities
- Assist clients with personal care tasks such as bathing, dressing, grooming, and hygiene.
- Support clients with mobility and transfers to ensure their safety and comfort.
- Monitor and report any changes in the client’s condition to the nursing staff or supervisor.
- Provide companionship and emotional support to clients, fostering a positive and caring environment.
- Assist with light housekeeping, meal preparation, and medication reminders as needed.
- Maintain accurate and timely documentation of care provided.
- Support household tasks such as meal prep and light housekeeping.
- Provide companionship and help clients remain safe and independent at home.
- Communicate changes in condition to the office promptly.
- Deliver care with dignity, patience, and professionalism.
