About The Position
As a Home Care Nursing Assistant (CNA) at Trinity Health at Home and Mercy Home Health, you’ll deliver one-on-one, high-quality care to patients in the comfort of their homes. Using advanced technology and your clinical expertise, you’ll assess, plan and manage individualized care that promotes healing and independence. We are a Catholic-based, non-profit organization, serving patients and their loved ones with home care (skilled nursing, physical/occupational and speech therapy and medical social work), palliative care, hospice and bereavement services. Our legacy continues with a pioneering, future-thinking care model. We blend clinical expertise with our exclusive Home Care Connect™ virtual care program to help patients achieve their health goals.
Requirements
- Graduate of an accredited nursing assistant program
- Active CNA Certification in the state
- Minimum of one (1) year of professional experience
