About The Position

The Home and Community Services Manager oversees the planning, coordination, and delivery of individualized services to individuals with disabilities in home and community settings. This role ensures compliance with all regulatory guidelines, supports the development of person-centered plans, and fosters a team environment to deliver exceptional care and support. We are seeking a leader who brings a strong continuous improvement mindset to their work. This role requires someone who actively identifies opportunities to enhance team performance, improve scheduling efficiency, and strengthen retention. The Home & Community Services Manager is expected to be proactive in addressing challenges, implementing solutions, and driving sustainable, positive change within their program.

Requirements

  • Minimum of 3 years of experience in managing services for individuals with IDD or similar populations.
  • Prior leadership experience is required.
  • Must be 18 years of age.
  • High School Diploma or GED is required.
  • Must successfully complete a pre-employment background check and any subsequent background check after employment.
  • Must have a valid driver’s license and be able to obtain a Missouri Class E chauffeur’s license within 14 days of hire.
  • Successfully complete employment-related training as directed.
  • Strong knowledge of home and community-based service models, person-centered practices, and Medicaid waiver programs.
  • Excellent interpersonal and communication skills.
  • Proven ability to manage multiple priorities and maintain organization in a fast-paced environment.
  • Proficiency in Microsoft Office Suite and other relevant software.
  • Strong ability to manage staffing operations, scheduling oversight, operational priorities, and team accountability.
  • Ability to address employee performance concerns professionally, consistently, and effectively.
  • Strong problem-solving, conflict resolution, and decision-making skills.
  • Ability to maintain professionalism, emotional regulation, and leadership consistency under pressure.

Nice To Haves

  • Associate or Bachelor’s degree in Human Services, Social Work, Healthcare Administration, or related field preferred.
  • Experience utilizing electronic scheduling, documentation, communication, and reporting systems preferred.

Responsibilities

  • Manages department operations of home and community-based services, ensuring services align with the organization’s mission and values.
  • Develop and implement person-centered service plans in collaboration with individuals, families, and interdisciplinary teams.
  • Monitor the quality of care provided and address any issues promptly.
  • Ensure compliance with federal, state, and local regulations, including Medicaid waiver guidelines.
  • Serve as the primary point of contact for individuals and their families, addressing questions, concerns, and service requests.
  • Advocate for the needs and preferences of individuals receiving services.
  • Coordinate with healthcare providers, case managers, and other community partners to ensure holistic support.
  • Develop and manage program budgets, ensuring fiscal responsibility.
  • Maintain accurate and up-to-date documentation, including case notes, incident reports, and service records.
  • Prepare reports on program performance, outcomes, and compliance metrics.
  • Participate in strategic planning and policy development for home and community services.
  • Provide direct supervision, leadership, coaching, and professional development support to Supervisors and assigned leadership staff.
  • Support leadership development by reinforcing accountability, professionalism, operational ownership, and communication expectations.
  • Conduct regular meetings, coaching sessions, observations, and performance discussions with Supervisors and staff.
  • Assist with recruitment, onboarding, training, scheduling oversight, retention efforts, and succession planning initiatives.
  • Promote a culture of accountability, professionalism, teamwork, and operational consistency throughout the department.
  • Maintain clear leadership boundaries while remaining approachable, supportive, and solutions-focused.
  • Support Supervisors in balancing operational leadership responsibilities with appropriate frontline engagement.
  • All other duties assigned.
