About The Position

The Home Care Registered Nurse provides nursing care according to established policies and procedures for a home care agency. The RN develops and supervises implementation of residents’ individualized service plans, observes their conditions, and communicates with physicians and families or responsible parties as needed. The RN oversees care delivered by the Home Support team, including LPNs, aides, home care workers, and companions. All clients are residents of the Oak Crest community, so no travel is required. The position offers PRN/flex opportunities, including weekends and an on-call rotation, at $48 per hour (approximately $70 per visit).

Requirements

• Current Registered Nurse license for the state of Maryland.
• Recent home care experience.
• Experience with the OASIS documentation system.

Nice To Haves

• Experience with NetSmart/MyUnity software.

Responsibilities

• Partner in initial evaluations to assess residents’ needs and develop and implement service plans with residents, families, physicians, and/or authorized practitioners.
• Communicate changes in residents’ conditions.
• Ensure residents’ individualized plans are implemented and followed by the Home Support agency team of providers.
• Deliver nursing care according to the plan of care and acceptable nursing standards.
• Participate in care conferences and other resident-related meetings.
• Participate in orientation and training of other agency nurses and in Aide staff development activities.
• Oversee care delivered by LPNs, aides, home care workers, and companions.
