About The Position

The Home Care Intake Specialist is responsible for initial client assessments, service planning, and coordination of care in compliance with AGE and agency guidelines across the 15 communities serviced by Bristol Aging & Wellness, Inc. Requirements

Bachelor’s degree in human services or a related discipline.
Excellent oral and written communication skills.
Ability to effectively relate to a wide variety of personalities.
Strong assessment skills.
Basic computer skills.
Organizational and time management skills.
Ability to multitask, adapt, and work in a fast-paced, high-volume work environment.
Driver’s license and reliable transportation required.

Nice To Haves

Experience in elder services is helpful.

Responsibilities

Ensure clients are assessed on a timely basis.
Conduct comprehensive evaluations to determine the full scope of clients’ needs.
Determine eligibility for programs.
Develop appropriate service plans that meet consumer choice and budget guidelines.
Partner with vendors to implement home care services.
Assist clients with problem solving and make referrals as needed.
Determine eligibility for public benefits.
Assist with applications for public benefits and health insurance, and resolve problems related to benefits.
Document phone calls, visits, and all activity in accordance with Elder Affairs’ and agency requirements.
Maintain accurate client records within the mandated timeframe.
Communicate with case management staff upon a case transfer.
